The most useful safety analysis is the kind an institution can’t do for itself. Too often, root-cause review is conducted by people who report, directly or indirectly, to those whose systems are under examination — which limits both depth and candor.
My work is independent and clinically deep. For 30 years I practiced anesthesiology across cardiac, transplant, neuro, and obstetric care; for nearly as long I’ve studied the systems around that care — directing educational informatics and teaching on medical error and high-reliability methods drawn from aviation and other domains, long before “patient safety” was a department. I bring that perspective, and current analytic and AI tools, to every engagement.
